Office Hours & Appointments

Our office is open Monday thru Thursday from 8 am to 4:30 pm, Fridays 8:00 am to 12:00 pm. The office reserves time each day for same day appointments.   Appointments can be made by calling 928-367-1077 after 9:00 am.

Nurse visits  i.e. blood pressure check, injections, laboratory tests are done Tuesday thru Thursday 8:00 am to 9:00am, no appointment necessary.

Appointment Cancellation

As a courtesy to other patients, we request office visit cancellations be called into the office at least 24 hours in advance.

What to Bring

Please bring the following to every visit:

  • Insurance card(s)
  • Photo ID
  • List of current medications and dosages
  • Medical records, lab or x-ray results related to your visit
  • Co-pay (we accept checks, cash, VISA, MasterCard, and American Express)

Patient Forms

Prior to the first visit, new patients are encouraged to print and complete Patient Forms.

Prescription Refill Policy

Requests for prescriptions are processed during regular business hours only.

Contact your pharmacy for all prescription refill requests.  The pharmacy will contact our office directly.  Allow 48 hours for our office to obtain authorization from your provider for the prescription refill(s) you have requested.

FDA controlled medications cannot be called into the pharmacy.  Patients will be required to pick up a signed prescription at the office during regular business hours.  Patients may be required to schedule an appointment in the office for medication management.

Billing Questions

Copays are due at the time of your visit.  A $25.00 fee will be assessed for returned checks.  Once a check has returned, you will be required to pay via cash or credit card.  All billing questions should be directed to Hall Resource Management (928) 535-6667. The billing office is open Monday through Friday from 8:00 a.m. – 5:00 p.m.

Once an account is placed in collection status, all prior balances must be paid in full before an appointment will be scheduled.  All future copays, deductibles/co-insurances will be due at the time of your visit or you will need to reschedule.
